ACCIDENTS - PROPERTY DAMAGE

PROPERTY AND EQUIPMENT DAMAGE

     When an accident occurs, advise FWMC Safety Department immediately. If repair costs or expenditures of any nature are anticipated, request a 600 series work order. Enter the number on the 600 report and complete in detail.

     FWMC’s Safety Department, maintain a file on all incidents in the event of further developments, legal action, or recovery from our insurance carrier.

     Please submit the 600 reports to FWMC’s Loss Control Department as quickly as possible. All Costs must be coded to the 600 work order number and a copy  of invoices submitted to the Loss Control Department.

REMEMBER TO TAKE PICTURES!

     It is wise to record all accidents and injuries in a diary and to maintain a job file. In the event of a lawsuit, this recorded information may be invaluable. Also, pictures!

GENERAL LIABILITY: INVESTIGATION AND CLAIMS

    Definition of general liability claims: General Liability claims are those claims regarding third party bodily injury or property damage involving any type of accident other than company-owned motor vehicles

(1)        Reporting

        a.         Call FWMC’s Loss Prevention Department and notify of the status of the claim.

        b.         Make written report within 24 hours of the accident.

(2)        Facts to obtain in accident investigation

        a.         Names of all people involved.

        b.         Description of exact location of accident.

        c.         If  company employees are involved in causing the accident, were they acting
                   within the scope of their duties.

        d.         Locate all witnesses to the accident and take notes of their version of the
                    accident.

         e.         List all names of possible people injured or owners of property damaged.

         f.          Observe if all warning signs and safety delineators were in place.

         g.         OBTAIN PHOTOGRAPHS of accident scene and the surroundings.

         h.         If accident was caused by equipment failure, do not use the unit until the
                     Loss Control Department can inspect it.
         I.          Make notes of general conditions of surrounding area, i.e. is area clean, or
                     caused by poor maintenance of steps, sidewalks, etc.
